Accounts that are 30 days or more past due will be assessed a 9% interest charge.
Cancellation Policy
Missed appointments or those cancelled within 48-hours of the originally scheduled appointment incur a fee of $15 for blood draws, vaccines and other lab appointments, $35 for IVs, $50 for office visits and $150 for missed procedures.

Changes To Your Appointment
As a courtesy, we utilize an automated appointment reminder system. Depending on the contact information you have provided us with and when you scheduled your appointment, you may receive multiple messages so as to provide you with ample opportunity to change an upcoming appointment if necessary.
However, keeping your appointment is still the patient’s responsibility, and therefore not receiving an automated reminder will not be an acceptable excuse for not showing to your appointment. Should you need to make changes to or cancel your appointment, please contact the office directly at 503-252-8125. In those instances, we do request you extend us the courtesy of a 48-hour notice. This courtesy allows us to continue to operate efficiently and use the time that was reserved for you to help other patients in need. Please note that for appointments that fall on Monday or Tuesday the cancellation must take place on Thursday of the week prior to your appointment. Voicemails or emails received to cancel appointments on the weekend do not count as 48 hour notice.
